Health Forms
- Please complete the required registration health form for the grade level your child will be entering in the fall.
- Remember to complete the Optional Health Forms in the event your child requires medication at school; needs to carry an asthma inhaler; has life-threateningg allergies; and/or plans to participate or tryout for an interscholastic athletic program (at Daniel Wright only)
- Please review the immunization requirements for the upcoming school year to ensure that your student has received what is required in order to start school on the first day of class.
- For all students enrolled at Daniel Wright Junior High School who participate in any of the interscholastic athletic events/sports, a physical examination is required. The physical can be completed on either the State of Illinois Certificate of Child Health Examination form or on the IHSA/IESA Pre Participation form and is valid for 395 days from the date of examination. All students wishing to participate in any sport must have the physical form on file with the school nurse prior to the start of tryouts.
Forms
Dental
All children entering Kindergarten, second-grade, and sixth-grade must present proof of having been examined by a licensed dentist before May 15 of the current school year. Failure to present proof allows the school to hold the child’s report card until the student presents proof: (1) of a completed dental examination, or (2) that a dental examination will take place by April of that school year.
Vision
All students entering Kindergarten, or upon first entry to an Illinois school must present proof of an eye examination performed within one year prior to entry into Kindergarten or the Illinois school. Failure to present proof by October 15 allows the school to hold the student’s report card until the student presents proof: (1) of a completed eye examination, or (2) that an eye examination will take place within 60 days after October 15.
Exemptions
A student will be exempted from the Dental Exam and Vision Exam requirements for: religious or medical grounds if the student’s parent/guardian presents to the school principal a signed statement explaining the objection; eye examination requirement if the student’s parent/guardian shows an undue burden or lack of access to a physician licensed to practice medicine in all of its branches who provides eye examinations or a licensed optometrist; or dental examination requirement if the student’s parent/guardian shows an undue burden or lack of access to a dentist.
