Contact the School Nurse
Nurses are in school when students are present. Please contact nurses when school is in session. Please see the following ways in which you may provide us with medical documentation:
- Fax to: Please fax all medical documents to our secured fax location at 847-821-0210. Please include a cover letter with your name and phone number, your child's name, grade level, school, and "Attn: Nurse". For your convenience, please use our medical cover letter when faxing.
- Mail to: Lincolnshire-Prairie View School District 103 Attn: Nurse, 111 Barclay Blvd. Suite 100, Lincolnshire, IL 60069
- Drop off at your child's school weekdays from 8 a.m. to 4 p.m.
