Health and Wellness Info and Forms
District 103 employs registered nurses who maintain up-to-date and accurate health records on each child in the school district. They provide staff training, basic medical care, and first aid, and they assist teachers in classroom health discussions. Their duties also include vision and hearing screenings for mandated grade levels and referred students. Students requiring ongoing individual health care may be eligible for individual health care plans and/or 504 Plans developed using district and federal protocols. All students are required to present appropriate proof that he/she received a health examination and the immunizations against, and screenings for, preventable communicable diseases within one year prior to:
- Entering Kindergarten or the First-Grade;
- Entering the Sixth-Grade; and
- Enrolling in an Illinois school for the first time, regardless of the student’s grade.
Included as part of the health examination is a diabetes screening (diabetes testing is not required) for students and a statement from a physician assuring “risk-assessed” or screened for lead poisoning (until the age of six years).
Evidence of immunity against the following diseases must be submitted at various grade levels: Polio, Diphtheria, Pertussis, Tetanus, Measles, Mumps, Meningicoccal, Pneumoccal, Rubella, Hepatitis B, Haemophilis Influenza Type B, and Varicella.
Failure to comply with the above requirements by the first day of the current school year will result in the student’s exclusion from school until the required health forms are presented to the school, subject to certain exceptions.
Exemptions: If a medical reason prevents a student from receiving a required immunization prior to the first day of school, the student must present, by the first day of school, an immunization schedule and a statement of the medical reasons causing the delay. The schedule and statement of medical reasons must be signed by an appropriate medical professional.

All students entering Kindergarten, or upon first entry to an Illinois school must present proof of an eye examination performed within one year prior to entry into Kindergarten or the Illinois school. Failure to present proof by October 15th allows the school to hold the student’s report card until the student presents proof: (1) of a completed eye examination, or (2) that an eye examination will take place within 60 days after October 15th. Exemptions: A student will be exempted from the Dental Exam and Vision Exam requirements for: religious or medical grounds if the student’s parent/guardian presents to the school principal a signed statement explaining the objection; eye examination requirement if the student’s parent/guardian shows an undue burden or lack of access to a physician licensed to practice medicine in all of its branches who provides eye examinations or a licensed optometrist; or dental examination requirement if the student’s parent/guardian shows an undue burden or lack of access to a dentist. All students with a diagnosis of asthma are recommended to have a rescue inhaler at school in case of emergency. There are 2 options available:
- A rescue inhaler can be kept in the health office. A medication authorization form must be filled out.
- Your child can self-carry the rescue inhaler. In order for your child to do so, the Parent Agreement for Child to Carry Asthma Inhaler must be returned to the health office with a copy of the prescription label.
In addition, all students with asthma are recommended to have an Asthma Action Plan filled out by their doctors and submitted to the health office yearly.
Students with a fever (higher that 100 degrees), who experience diarrhea or vomiting should remain at home until being symptom free for 24 hours. Students with undiagnosed rashes may be sent home and referred to their healthcare provider for diagnosis and would require a note upon return.
PE Exclusion Guidelines: In the event of illness or injury, a parent request to excuse a student from participation in physical education class and recess will be honored for a maximum of two days. If further recovery is needed, a note from the student's healthcare provider is required.
For all students enrolled at Daniel Wright Junior High School who participate in any of the interscholastic athletic events/sports, a physical examination is required. The physical can be completed on either the State of Illinois Certificate of Child Health Examination form or on the IHSE/IESA Pre Participation form and is valid for 395 days from the date of examination. All students wishing to participate in any sport MUST have the physical form on file with the school nurse PRIOR to the start of tryouts. Locate the IHSA/IESA form here.
In addition to the physical examination requirement, the Permission to Participate in Extracurricular Activities form must be completed and on file prior to trying out.
Vision and hearing screenings are conducted by our school nurses according to State of Illinois mandates. When results indicate a student has failed to meet specific criteria, the school nurse provides written referral/screening results to the parents along with directions on next steps.
